I need a medically-savvy writer who can turn the raw clinical information I provide into clear, concise patient progress notes. Each note must focus on two core elements: the current treatment plan and all medications (dosage, route, frequency, and any recent changes). I’m not looking for full patient histories or extensive diagnostic discussions; the emphasis is squarely on documenting ongoing management in a way that would satisfy an EMR audit and keep the care team instantly up to date.
You’ll receive de-identified intake sheets or voice recordings, along with any changes that occurred during the shift. Using a standard SOAP or narrative style—whichever best fits the case—you will:
• Translate my shorthand into complete, professional-grade progress notes.
• Ensure terminology is correct and consistent with current clinical guidelines.
• Return notes in Word or directly in the EMR interface we provide (Epic-style formatting preferred, but I’m flexible).
Accuracy and turnaround matter more than volume; I’d like same-day delivery for anything sent before 3 p.m. local time. All material is HIPAA-sensitive, so experience handling protected health information is essential.
